Controlling high blood pressure and helping people lead healthier lives
Take charge of your heart health with our new Healthy Hearts Ambassador Blood Pressure Self-Monitoring (HHA-BPSM) program! This free, evidence-based program empowers adults with high blood pressure to track and manage their numbers from home with personalized support.
Over a four-month period, participants receive a free home blood pressure monitor and learn how to accurately measure and record their readings.
You’ll meet regularly with a trained Healthy Hearts Ambassador, who will guide you through self-monitoring, review your progress, and offer encouragement and tips for healthy living.
Adults 18 and older who have been diagnosed with high blood pressure or take blood pressure medication (Participants should not have had a heart attack within the past 12 months, irregular heartbeat, or lymphedema).
