Health Department Issues Request for Proposals (RFP) for Independent Evaluation Consultant

LEONARDTOWN, MD (November 4, 2026) – The St. Mary’s County Health Department (SMCHD) is issuing this RFP (27-06) to seek proposals from qualified independent evaluation firms, organizations, or consultants to conduct a comprehensive evaluation of the St. Mary’s 360: Connected Care Everywhere project.

St. Mary’s 360 is SMCHD’s initiative to establish an integrated, technology-enabled system of care that connects residents to primary care, behavioral health, oral health, public health services, wellness supports, emergency response systems, and resources that address health-related social needs. The initiative centers on a Community-Integrated Care Coordination system using embedded care coordinators, closed-loop referral technology via the Health & Wellness Portal, digital health supports, mobile integrated health outreach, and multidisciplinary care planning.

SMCHD is seeking an independent evaluation partner to develop and implement a public health mixed-methods evaluation and continuous quality improvement approach to assess both the implementation and outcomes of St. Mary’s 360. The evaluation is expected to draw on multiple quantitative and qualitative data sources, including Health & Wellness Portal analytics, CRISP resources, participant and partner feedback, and other available data.

Eligible applicants may include, but are not limited to:

  • Independent consulting firms
  • Research organizations
  • Academic or university-based research centers
  • Nonprofit organizations
  • Other entities with demonstrated expertise in public health, healthcare, health services, population health, or related program evaluation.

To be eligible for consideration, applicants must:

  • Demonstrate experience designing and conducting mixed-methods evaluations
  • Demonstrate experience evaluating public health, healthcare, health and human services, community-based initiatives, or other complex programs involving multiple partners or service delivery systems
  • Demonstrate capacity to analyze and interpret quantitative program, utilization, referral, clinical, administrative, or other health-related data
  • Demonstrate capacity to develop evaluation frameworks, performance measures, data collection methodologies, and actionable recommendations that support continuous quality improvement.

A Pre-Proposal meeting will be held via Google Meet on November 09, 2026 at 1:30 PM Eastern Standard Time.

Proposals submitted in response to RFP #27-06 Independent Evaluation Consultant must be submitted by 3:00 PM Eastern Standard Time on November 24, 2026. For more information or to submit a proposal for this request, please visit smchd.org/rfp.
