LEONARDTOWN, MD (August 27, 2026) – The St. Mary’s County Health Department (SMCHD) is seeking proposals from qualified organizations to design, expand, or implement mobile, home-based, community-based, or place-based health access strategies for residents of St. Mary’s County.
For purposes of this RFP, “mobile health” is interpreted broadly. It may include traditional mobile health units, home-based services, community-based micro-access points, telehealth access points, digital health navigation, transportation-enabled access strategies, remote patient monitoring, Community Health Worker (CHW)/Certified Peer Recovery Specialist (CPRS)/peer outreach models, and other strategies that bring health care, care coordination, or health-related supports closer to residents.

SMCHD will prioritize applicants that are locally or regionally anchored and have the operational, fiscal, and compliance infrastructure needed to manage public funds.
A virtual overview and Q&A for this RFP will be held on September 4, 2026, at 12:00 PM Eastern Standard Time. Proposals submitted in response to RFP #27-05, Mobile Health Services Expansion Initiative, must be submitted by 5:00 PM Eastern Standard Time on September 17, 2026.
