Health Department Encourages Local Organizations to Submit Applications for State’s Rural Health Transformation Program Funding

LEONARDTOWN, MD (May 1, 2026) – The St. Mary’s County Health Department encourages eligible local organizations to submit applications for grant funding to the Maryland Department of Health (MDH) Request for Applications (RFA) related to the Rural Health Transformation Program (RHTP)

MDH has issued the RHTP RFA for organizations serving rural communities, with a focus on Sustainable Access through Service Expansion and Health Information Technology, Behavioral Health Services Expansion, and Innovative Care. 

MDH aims to award RHTP funding to applications with the goal of:

  • Expanding primary care, specialty care, and behavioral health care to meet current demand and build long-term capacity to improve health outcomes.
  • Promoting efficient and effective management of chronic somatic and behavioral health conditions, including by empowering residents to manage their conditions.
  • Leveraging technology to bring health care directly to rural residents.
  • Empowering residents to manage somatic and behavioral chronic conditions.
  • Helping providers adopt innovative, value-based care models.
