Requests for Proposals: Pathways to Health Equity 

LEONARDTOWN, MD (July 21, 2022) – The St. Mary’s County Health Department (SMCHD) Behavioral Health Division is seeking two contractors to provide services for the Pathways to Health Equity program.

SMCHD is seeking a Youth Mentor Program for the period of two (2) years, beginning September 1, 2022 through April 30, 2024, to provide youth academic support and mentoring services. Communities funded as part of Pathways to Health Equity program are required to implement the five steps of the Strategic Prevention Framework (Assessment, Capacity, Planning, Implementation, and Evaluation) in their efforts to reduce the number of behavioral health related emergency department visits, reduce emergency department utilization for preventable complications from diseases, and mitigate negative social determinants of health the in service area. Funding available for the Youth Mentor services is $12,500 each year for a total of $25,000.  The deadline to submit a proposal for this contract is August 19, 2022.

SMCHD is also seeking an Evaluator for the period of two years, beginning August 1, 2022 through April 30, 2024, to provide evaluation services to support the Pathways to Health Equity program for the St. Mary’s County Health Hub. This process will require a significant evaluation effort by the local health department and its corresponding community coalitions. Funding available for the Evaluator services is $25,000 each year for a total of $50,000.  The deadline to submit a proposal for this contract is August 19, 2022.
