A care coordination program for pregnant and postpartum individuals (up to 1 year postpartum) and children aged 0–3 with health concerns or needs for community resources and support. By leveraging coordinated care, community partnerships, and targeted interventions, the program aims to improve maternal and infant health outcomes in St. Mary’s County and reduce disparities in perinatal care access and quality.
Thrive By Three helps pregnant and postpartum individuals and children ages 0–3 in St. Mary’s County access the care and support they need. Families receive personalized help with health education, screenings, referrals, insurance, and connections to local resources and services. The program offers flexible support through in-person, phone, and video visits, tailored to each family’s needs. Participants also receive guidance on safe sleep, essential items like pack-and-plays and car seats (while supplies last), and follow-up support after birth. We aim to promote healthier pregnancies, stronger families, and a bright start for every child.
This program is geared to serve pregnant and postpartum individuals up to one year postpartum, as well as families with children between the ages of birth and three, who need local support and/or services. Participants must live in St. Mary’s County. Enrollment is voluntary and can be done by the individual or their provider.
